We have been working with health colleagues locally on a project to improve the experience of families when children move from children’s to adult NHS health services.
We have had consistent feedback over the years that health transitions need to be improved. As part of Bolton’s Preparing for Adulthood (PFA) Forum, there is a workstream that focuses on Health and PFA and this is where we have been working on this project.
If you would like to read about the project so far click here The project is now at a point where we have coproduced a set of 4 principles, that we would like to share with you and ask for your opinions. The principles would be the start of a wider project to improve the transition process, so it doesn’t stop here. We wanted to start by working out what the main focus should for families, and that is how the principles have been developed.
We’d like to hear from you, it doesn’t matter how old your child or young person is, as ling as they are between 0-25! And it doesn’t matter how much you use health services, we welcome broad opinions. Our short questionnaire introduces the principles and asks you how important that principle is to transition.
